1. Acknowledgement, Release, Covenant and Indemnification Agreements 
2. Honduras Outreach Missioner Profile
3. Physician's Statement
4. A legible copy of your passport, valid for six months after your return date
5. A legible copy of your round trip air travel arrangements.
6. A check made out to "HAVE Foundation" for the in-country fee of $720.00
